Pre-Employment Investigation and Specific Release

I certify and declare under penalty of perjury under relevant state and federal law that the information contained in my employment application is complete, true and accurate. I acknowledge that falsification or omission of information may result in immediate dismissal or retraction of any offer of employment.


In consideration of Champion Maintenance & Construction LLC review of my application for employment, (herein referred to as the employer) I hereby voluntarily consent to and authorize employer or its authorized agents bearing this release or copy thereof to obtain a consumer report for employment purposes. I agree that this consumer report may include any of the following:


  • Employment Verification, Education Verification, Credentials Verification, and Military Service Verification
  • Personal Identity Verifications, Past Employment Verifications, and Reference Checks
  • Criminal Records, Civil Cases, Motor Vehicle Records, and Credit Report

I authorize all persons and organizations that may have information relevant to this research to disclose such information to employer or its authorized agents. I hereby release employer, its authorized agents, and all persons and organizations providing information from all claims and liabilities of any nature in connection with this research. I hereby further authorize that a photocopy of this authorization may be considered as valid as the original.


I understand that I have specific prescribed rights as a consumer under the federal Fair Credit Reporting Act (FCRA) and may have additional rights under relevant state law. I hereby certify that I have been presented with a summary of my rights as a consumer under the Fair Credit Reporting Act (FCRA).

A. Sexual Harassment Policy

All employees should be aware of the fact that Champion Maintenance & Construction LLC has a specific Company Policy that explicitly prohibits sexual harassment of any nature in the work place. Champion Maintenance & Construction will also make every effort possible to provide a work environment that is free from sexual comments, jokes, materials, or discussions of sexual topics. These types of activities are offensive to most employees and will not be allowed.

If any Champion Maintenance & Construction employee feels that they are being sexually harassed, it should be immediately reported to their Supervisor. Employees should feel free to report these incidents to other management personnel.

This type of conduct by an employee may prove to be grounds for immediate dismissal.

Todo empleado debe estar informado que la compania tiene una politica clara prohibiendo el acoso sexual de cualquier tipo en el trabajo. La compania hara todo lo’ que este a su alcance para mantener un ambiente de trabajo libre de comentarios sexuales, chistes, objectos o conversaciones de temas de sexo. Este tipo de actividades son ofensivas para la mayoria de empleados y no sera tolerada.

Todo supervisor debe ser notificado de immediato, si algun empleado en esta compania siente acoso sexual. Los empleados deben tener la confianza de reportar este tipo de incidente a la gerencia.

Este tipo de comportamiento de un empleado puede resultar en despedida immediata.

B. Rules & Regulations | Reglas Y Reglamentos

  • 1. Each employee must sign in his or her own name, in and out every day and must be on time.
  • 2. If you are going to be out for the day you must call the office to let us know at least by 2:00 PM that day.
  • 3. You are responsible for returning keys each night to the designated area.
  • 4. Use of telephones in any office is not permitted and will call for automatic dismissal.
  • 5. Smoking is not permitted in any building.
  • 6. Children are not permitted at any time and are not allowed to be outside of the buildings.
  • 7. You are responsible for your belongings. They must be left in one place and taken out when you leave the building.
  • 8. You are not permitted to take anything out of the building.
  • 9. Work behind locked doors and admit no one into building at any time.
  • 10. Throw out only what is in the waste baskets. Take boxes that are marked with BASURA labels only.
  • 11. DO NOT lean anything against painted walls. DO NOT hold doors open with broomsticks.
  • 12. Turn off all lights and lock doors after cleaning.
  • 13. Close all windows after cleaning.
  • 14. DO NOT hit furniture with vacuum.
  • 15. Report all doors found unlocked.
  • 16. DO NOT open any doors for anyone. They must have their own keys.
  • 17. If you break anything, let your supervisor know.
  • 18. If you find anything wrong in the space let your supervisor know at once.
  • 19. In case of fire or emergency, call 911, call your supervisor and call the office at once.
  • 20. Treat your fellow employees with respect.

  • 1. Cada empleado debe firmar su propio nombre de entrada y salida todos los dias, y estar a tiempo.
  • 2. Si va a faltar el dia, debe llamar a la compania para dejarnos saber por los menos a los 2:00 PM de ese dia.
  • 3. Usted es responsable de devolver llaves cada noche a la area designada.
  • 4. El utilizar telefonos en cualquier oficina no es permitido, y es causa automatica de despido.
  • 5. No es permitido fumar en cualquier edificio.
  • 6. Ninos no estan permitidos en ningun momento, tampoco estar afuera de los edificios.
  • 7. Usted es responsable por sus pertenencias, deben quedar en un solo lugar y sacarlos cuando deja el edificio.
  • 8. No es permitido sacar nada fuera del edificio.
  • 9. Trabajar detras de puertas con seguro, y no permitir a nadie en el area.
  • 10. Tirar lo que este dentro del tacho de BASURA unicamente. Sacar cajas que esten marcados con etiquetas de BASURA solamente.
  • 11. No recostar nada contra superficies pintadas. No utilizar palos de escobas para mantener puertas abiertas.
  • 12. Apagar todas las luces y cerrar con llave todas las puertas despues de limpiar.
  • 13. Cerrar todas las ventanas despues de limpiar.
  • 14. No golpear muebles con la aspiradora.
  • 15. Reportar si encurtra filguan puenta sin seguro.
  • 16. Reportar todas las puertas que encuentre sin seguro.
  • 17. No abrirle las puertas a nadie.
  • 18. Si rompe algo, o no encuentra todo bien en su espacio for favor notificarle a su supervisor immediatamente.
  • 19. En caso de fuego o emergencia, llamar al 911, a su supervisor y a la oficina enseguida.
  • 20. Tratar a sus companeros de trabajo con respeto.
